where is the 2nd intercostal space

by Krystal Block Published 3 years ago Updated 3 years ago

2 nd intercostal space starts from inferior portion of 2 nd rib and ends at superior portion of 3 rd rib. The intercostal spaces are the spaces amongst the two bordering ribs and their costal cartilages. On each side 11 intercostal spaces are present.

From the angle of Louis, move your fingers to the right and you will feel a gap between the ribs. This gap is the 2nd Intercostal space.

Full Answer

How do you find the 2nd intercostal space?

Where is the 2nd intercostal space? If you find the sternal notch, walk your fingers down the manubrium a few centimeters until you feel a distinct bony ridge. This is the sternal angle. The 2nd rib is continuous with the sternal angle; slide your finger down to localize the 2nd intercostal space.

Where is the intercostal space located?

Intercostal spaces lie between adjacent ribs and are filled by intercostal muscles ( Fig. 3.26 ). Intercostal nerves and associated major arteries and veins lie in the costal groove along the inferior margin of the superior rib and pass in the plane between the inner two layers of muscles.

Where is the second intercostal space on the human volunteer?

Only 15 (60%) correctly identified the second intercostal space on the human volunteer, all placing the needle medial to the midclavicular line, with a range of 3 cm. Two (8%) named and identified the site of needle pericardiocentesis; one (4%) named and identified the fifth intercostal space in the anterior axillary line.

What is the 6th intercostal space used for?

Intercostal spaces (ICS) are an important access portal for entering the pleural space for thoracentesis and chest tube insertion. On the midaxillary line, in the 6th intercostal space. Note: According to some texts, this point is located in the 7th intercostal space.

Is the second intercostal space above or below the rib?

The intercostal space (ICS) is the anatomic space between two ribs (Lat. costa). Since there are 12 ribs on each side, there are 11 intercostal spaces, each numbered for the rib superior to it....Intercostal spaceLatinspatium intercostaleTA98A02.3.04.007TA21102FMA122435 more rows

Where is the 2nd rib located?

The second rib attaches to the sternum at the sternal angle. Since the first rib is hidden behind the clavicle, the second rib is the highest rib that can be identified by palpation. Thus, the sternal angle and second rib are important landmarks for the identification and counting of the lower ribs.

Where is the intercostal spaces located?

between the ribsThe intercostal spaces, also known as interspaces, are the space between the ribs. There are 11 spaces on each side and they are numbered according to the rib which is the superior border of the space.

What is called space between two ribs?

The intercostal spaces, also known as interspaces, are the space between the ribs. There are 11 spaces on each side and they are numbered according to the rib which is the superior border of the space.

What intercostal space is the diaphragm?

At rest the diaphragm rises to the level of the fourth intercostal space on the right and the fifth intercostal space on the left. At maximal contraction the diaphragm descends two rib spaces bilaterally.

Where is 4th intercostal space?

The location of the 4th and 5th intercostal space is related to the length of the sternum. It is 77% of the sternal length that measures 15 cm for the 4th intercostal space. The position of the V1 and V2 electrodes decreases to 57% when the sternal length is 26 cm.

Where is 5th intercostal space?

V4: Fifth intercostal space, in the same vertical line as the clavicle (collarbone).

What are the layers of the intercostal space?

There are 3 muscular layers in each intercostal space, consisting of the external intercostal muscle, the internal intercostal muscle, and the thinner innermost intercostal muscle. These muscles help to move the ribs during breathing.

Which bundle runs high in the intercostal space?

The neurovascular bundle has a strict order of vein - artery - nerve (VAN), from top to bottom. This neurovascular bundle runs high in the intercostal space, and the smaller collateral neurovascular bundle runs just superior to the lower rib of the space (in the order NAV from superior to inferior).

Which muscle is covered by the parietal pleura?

Nerves. In reference to the muscles of the thoracic wall, the intercostal nerves and vessels run just behind the internal intercostal muscles: therefore, they are generally covered on the inside by the parietal pleura, except when they are covered by the innermost intercostal muscle s, innermost intercostal membrane, ...

What is the intercostal space?

INTERCOSTAL SPACE. Each intercostal space from superficial to deep, has two layers of muscle; an artery, a vein, and a nerve; and a diminutive inner layer of muscle. The external intercostal muscles run obliquely with fibers in the same orientation as the external oblique muscle of the abdomen (fingers in pockets).

What is the rib cage?

The rib cage. The rib cage, which consists of 12 pairs of ribs and the sternum, provides protection for the heart and lungs and facilitates breathing. The sternum is a fairly flat bone and the ribs are also fairly flat in cross section. As the ribs form the major part of the rib cage, they have a distinct curved shape.

Where are the innermost intercostal muscles located?

The innermost intercostal muscles are located deep to the neurovascular bundle and run in the same direction as the internal intercostal muscles. While mentioned in anatomy texts, surgically, the innermost intercostal muscles do not need to be considered separately from the internal intercostal muscle (Figure 14).

Which artery enters the sixth intercostal space?

In the sixth intercostal space it divides into its two terminal branches, the musculophrenic and superior epigastric arteries, the latter entering the anterior abdominal wall by passing through the diaphragm.

What are the ribs attached to?

The upper seven pairs of ribs are attached to the sternum by separate costal cartilages and are therefore sometimes referred to as true ribs. The costal cartilages of the eighth, ninth and tenth pairs of ribs fuse with each other before fusing with the costal cartilages of the seventh ribs ( Figure 3.13 ).

What is the shape of the ribs?

As the ribs form the major part of the rib cage, they have a distinct curved shape . The heads (posterior ends) of the ribs articulate with the thoracic vertebrae. The anterior ends of the upper 10 pairs of ribs are attached to the sternum by pieces of cartilage called costal cartilages (costa = rib).

What are the lower two pairs of ribs called?

The lower two pairs of ribs do not attach onto the sternum; the anterior ends of these ribs are free, and they are referred to as floating rib s. Because none of the lower five pairs of ribs has a direct cartilaginous or other attachment to the sternum, these ribs are sometimes referred to as false ribs.

How many pairs of thoracic spinal nerves are there?

The 12 pairs of thoracic spinal nerves intervates the thoracic wall. When they leave, the intervertebral foramina they split into anterior and posterior rami. The anterior primary rami of upper 11 thoracic spinal nerves (T1-T11) are named intercostal nerves as they course via the intercostal spaces.

What are the atypical nerves?

Atypical intercostal nerves (1st, 2nd, 7th, 8th, 9th, 10th, and 11th). The typical intercostal nerves are those which remain confined to their own intercostal spaces. The atypical spinal nerves extend beyond the thoracic wall and partly or entirely supply the other regions.

What is the layer of connective tissue that separates the ribs and intercostal spaces?

Deep to the intercostal spaces and ribs, and separating such structures from the underlying pleura, is a layer of loose connective tissue, knowned as endothoracic fascia, which consists of variable amounts of fat. Superficial to the spaces are deep fascia, superfcial fascia, and skin.

What is the intercostal space?

Contents. The Intercostal space (ICS) is the space in between the two adjacent ribs. There are 11 intercostal spaces on each side, each intercostal space is numbered for the rib superior to it. Every space consists of intercostal muscles and neurovascular bundle of intercostal plane. In each space, the vein is the most superior structure ...

How many intercostal veins are there?

The number of intercostal vein corresponds to the number of intercostal arteries, i.e., each intercostal space contains two anterior intercostal veins and one posterior intercostal vein. Their tributaries correspond to the branches of the arteries.

What is the 3rd 6th space?

The blood supply and nerve supply of 3rd-6th intercostal spaces is restricted only upto the thorax as a result of which these 3rd-6th spaces are typical intercostal spaces. Intercostal Spaces.

Which artery provides the thoracic wall with blood?

Intercostal Arteries. The thoracic wall has rich blood supply. It is provided by the posterior and anterior intercostal arteries. Each intercostal space contains one posterior and two anterior intercostal arteries (upper and lower).

What Article are we Reviewing Today?

Laan DV et al. Chest Wall Thickness and Decompression Failure: A systematic Review and Meta-Analysis Coparing Anatomic Locations in Needle Thoracostomy. Injury 2015 [Epub Ahead of Print]. PMID: 26724173

What They Did

Systematic review and meta-analysis of trials assessing chest wall thickness and needle thoracostomy effectiveness

Strengths

Adhered to the Preferred Reporting Items for Systematic Reviews and meta-analyses (PRISMA)

Limitations

Regional variability in chest wall thickness may be partially explained by differences in BMI, thus in the USA and other western countries where BMI is higher, a change may be necessary in needle thoracostomy location or catheter length, but in other parts of the world, ICS2-MCL decompression may still be appropriate

Clinical Take Home Points

Best evidence suggests that the 4 th /5 th ICS-AAL has the lowest predicted failure rate of needle decompression with a 5cm angiocatheter

What is the 2nd rib?

The 2nd rib is continuous with the sternal angle; slide your finger down to localize the 2nd intercostal space. The angle of Louis also marks the site of bifurcation of the trachea into the right and left main bronchi and corresponds with the upper border of the atria of the heart.

How many pairs of ribs are attached to each other?

The lower 5 ribs do not attach to the sternum. The 8th, 9th, and 10th ribs are attached to each other by costal cartilage.

What is the angle of Louis?

For example: The angle of Louis (also called the sternal angle) is a useful place to start counting ribs, which helps localize a respiratory finding horizontally. If you find the sternal notch, walk your fingers down the manubrium a few centimeters until you feel a distinct bony ridge. This is the sternal angle.

What are anatomic landmarks?

Anatomic Landmarks. It is important to review the anatomy of the chest wall and thoracic cavity, as you will use anatomic landmarks to document the location of respiratory assessment findings. The thoracic cavity is made up of 12 pairs of ribs that connect in the posterior thorax to the vertebral bodies of the spinal column.

What are the 11th and 12th ribs?

The 11th and 12th ribs, known as “floating ribs,” are not attached in any way to the sternum; they move up and down in the anterior chest , allowing for full chest expansion. Please review the important landmarks of the bony thoracic anatomy.

Abstract

Background: Invasive practical procedures require identification of surface anatomical landmarks to reduce risk of damage to other structures. Needle thoracocentesis has specific complications, which have been previously documented.

METHODS

An observational study was performed in order to investigate the knowledge and skills in identifying surface landmarks for needle thoracocentesis amongst emergency physicians. The grade of doctor and ATLS status were recorded.

RESULTS

Twenty five emergency physicians were included: six senior house officers, five staff grades, six specialist registrars, and eight consultants. Twenty two of the participants had completed ATLS training within the previous 10 years.

DISCUSSION

The primary aim of this study was to describe the relation between knowledge of the preferred site of needle thoracocentesis and the ability to correctly identify this point on a human volunteer.

What is the name of the narrowing of the mitral valve?

Mitral stenosis (MS) is narrowing of the mitral valve, which results in decreased filling of the left ventricle during systole and increased left atrial pressure (due to incomplete left atrial emptying).

What is the term for a tightening of the aortic valve?

Aortic stenosis. Aortic stenosis (AS) refers to a tightening of the aortic valve at the origin of the aorta. Aortic stenosis is associated with an ejection systolic murmur heard loudest over the aortic valve .

How many cusps does the aortic valve have?

Congenital abnormality of the aortic valve: the aortic valve is normally composed of three cusps (known as a tricuspid valve), but in some cases, individuals have only two cusps (known as a bicuspid valve) which predisposes them to the development of AS as well as aortic regurgitation.

What is MR in a heart?

Mitral regurgitation (MR) occurs when there is backflow (regurgitation) of blood from the left ventricle into the left atria (through the mitral valve) during ventricular systole. Mitral regurgitation is associated with a pansystolic murmur heart loudest over the mitral area and radiating to the axilla.

What causes the first heart sound?

The first heart sound (S1) is caused by the closure of the mitral and tricuspid valves. It marks the start of ventricular systole, and a peripheral pulse is felt at the same time (or shortly after) S1.

What is the contraction of the ventricular systole?

This phase of ventricular systole is called isovolumetric contraction. Eventually, the pressure within the ventricles exceeds the pressure in the pulmonary artery and aorta causing the pulmonary and aortic valves to open. Blood is ejected from the ventricles during ventricular ejection phase.

What causes the aortic and pulmonary valves to close?

The drop in pressure within the ventricle causes the aortic and pulmonary valves to close, to prevent backflow (regurgitation) of blood into the ventricles. Figure 1. An overview of the cardiac cycle. 1. For more information, see the Geeky Medics guide to the electrical conduction system of the heart .
