The safety data surrounding iEPO in the setting of refractory hypoxia in ARDS is also limited due to the lack of published data. Many safety data are derived from small retrospective trials and case reports. 6,11 Most have reported limited to no adverse reactions associated with iEPO. I.V.
Nursing considerations
When clinicians are considering iEPO for the treatment of refractory hypoxemia in severe ARDS, the choice of dose initiation and titration methods are extremely important to reach desired oxygenation goals. High doses of iEPO do not show significant improvement in oxygenation from baseline.
Conclusion
Overall, there is insufficient supporting evidence for routine use of iPVDs in patients with ARDS. iEPO is as effective and less costly than iNO, although standardized dosing and administration techniques have not yet been established.
