Centric occlusion is the occlusion a person has while in centric relation. For most people, I believe, maximum intercuspation or maximum closure or habitual occlusion, these are all synonyms, is not centric occlusion because most people do not bite down in centric relation, although it is possible.

What materials are used to record centric relation?
Materials available for recording centric relation are (1) wax; (2) modeling plastic; (3) quick-setting impression plaster; (4) metallic oxide bite registration paste; (5) polyether impression materials; and (6) silicone impression materials. Of these, wax is likely to be least satisfactory unless carefully handled. If not uniformly softened when introduced into the mouth, it can record a position with unequal tissue placement. Also, it does not remain rigid and dimensionally stable after removal unless carefully chilled and handled upon removal (Figure 12-12 ).

Can a patient have an exacting centric relation occlusion?
An exacting centric relation occlusion is rarely possible in the maxillofacial patient who has any anomaly, trauma, or resection that involves the muscles of mastication and affects the envelope of motion. A repeatable hinge motion of the mandible is not evident; rather a deviated and rotated path of motion is the norm. An articulator cannot mimic this motion (Hanasono and Sevallos, 2010 ). A slight change in OVD can equate to a much greater change in deviation and rotation. The clinician is less worried about where the mandible is when the mouth is open, but rather has functional concerns upon final contact with the opposing dentition. There is a philosophy that to decrease the deviated position of the mandible at contact and realign the facial form of the chin point, the clinician should guide or push the mandible into the appropriate closing motion and final occlusal contact. Unfortunately, this guidance places the mandible in a position that the patient may not be able to obtain during his or her own closure, because the remaining muscles and tissues have their own envelope of motion and closure. The patient does have proprioception and the envelope of motion, albeit aberrant due to lack of bilateral muscle function, can often be repeatable within an occlusal contact area of a few millimeters. Head position will alter the posture of the mandible and closure of the mandible, again because of lack of bilateral muscle function. Therefore, making a record for occlusal restoration using a head upright position at the OVD which is patient directed rather than clinician forced has the greatest likelihood of ensuring patient comfort. Functionally generated occlusion may be valuable.

This study investigates the differences in condylar position between centric relation (CR) and centric occlusion (CO) in Japanese orthodontic patients before treatment. We employed 150 consecutive cases (age range: 6-57 years) for the study. Dental casts were mounted on a Panadent articulator with a power centric CR bite record.
