How do I add an image to a Jira comment?

Disable the JIRA Drag and Drop Attachment Plugin, from Manage add-ons. The plugin can be found under "System add-ons".
After which you may view an issue and click on the '+' (plus) sign to add an attachment, as shown below.
A screen will pop up to attach files and include a comment, as shown below:
