How hard is NBME 18 compared to the real thing?
247 on NBME 18, 246 on the real thing. Honestly, the real thing felt considerably easier than NBME 18. I just didn't let myself say it until today because I didn't want to tempt fate.
Is my NBME score on form 18 predictive?
My NBME average is ~ 10 points lower than my score on form 18, so I'm inclined to think its an outlier and probably not predictive. Yale thing? Trust in your averages. NBME 18 was 100% "predictive" in the sense that I got a 262 on 18 and a 262 on the real thing. However, my average across NBME 15 - 18 was also a 262.
Should I take NBME 13 or 16?
Hey for what it’s worth, 13 is quite different from the rest of the NBMEs. Don’t get too discouraged, 16 and 17 are much better/more reflective of Uworld from my experience thus far. One week to go, taking NBME 18 in a couple days.
Is the curve for NBME 23 worth it?
The curve is absolutely nuts. ~85% is ~230. The questions are easier to compensate, but this just means you're penalized higher if it focuses on your weak areas. Finally, to speak on how predictive it is, the predictor formula gave it an R2of .589 whereas NBME 22 has an R2of .618 and NBME 23 has an R2of .620.

Because the scoring system for the USMLE Step 1 (and by extension the NBME CBSSA) is essentially a black box, we cannot know for sure how predictive these exams will be.
