Truck threw the code for throttle body sensor (3rd time this has happened in 90k miles of driving). I decided to clean it and it was absolutely filthy considering Throttle body was new 30k ago (pic below). Cleaned the MAF sensor and air filter while I was at it. Put it all back together, code is gone, but I can’t get it to idle correctly again.

The Tb relearn is more a idle and drive control to learn the air value. I would also clean the intake tube and the two valve cove vents. Sorry but have to ask....do you have a catchcan ? If so not on the valve cover breather tubes? Also makes me wonder how your maf is doing... Oh and you do not need a any tools for the tb relearn.

In the tune it says pedal % is what "resets" the tb air flow tables. So as stupid as it sounds you need to wot then let off. Then it can calculate max air flow. In tune we reduce that relearn when we have big cans so we get a recalibration before we data log for fuel tables. Looks like oil....if you work in dust alot maybe a pre filter would help.

Go back to OEM intake into the fender and use the Heavy Duty AC Delco paper filter. That is all outside contamination adhering to the TB.

If you have any kind of decent filter that is not dirt/dust from outside IMO. That looks like oily gunk from the engine. The light volotiles go through the engine leaving oily film behind. A catch can or as suggested venting thru a filter to atmosphere would help IMO. Try one of those and you should quickly tell the difference.
